I have done many mysql upgrades and normally there will not be any issues. Only thing you need to make sure is you have proper mysql dumps for all your databases including PSA for sure. Upgrades are Ok but downgrades may cause issues and not suggested.

So some database inconsistency may have happened. It may require a detailed investigation and better to contact your server administrator or service provider for the same
If you are not sending any bulk mails, then submit a whitelist request and that should fix it . It will take some time to pick up the change in dns and rdns made and that may have caused this.
Are you able to export a dump from plesk interface ? If not then the db may have some how got removed but still I wonder if that is the case then how the site is working.

Seems you changed the hostname and that fixed I hope . I just revisited that and can see the hostname is fixed and is with www . Can you please confirm
