Issue Apache (or nginx reverse) do only run on ipv6 after a few minutes (fail2ban?)

I have no idea why but since today my webserver is basically ipv6 only.

I already tried to remove the nginx reverse proxy and even removed ipv6 (hoped to get an error message, but got nothing). I also tried the plesk repair tool.

The only solution is disabling Fail2ban (it runs at least now for 30 minutes).

Any idea why and what is going on?

- Can you reach your server by your IPv4 address at all? E.g. if not www, then maybe ftp, ssh?
- Is your IPv4 address being blocked by Fail2Ban? In that case, have you added it to the Fail2Ban whitelist?
- Does that address appear anywhere in /var/log/fail2ban.log?
