According to this site: http://blog.unmaskparasites.com/2012/06/22/runforestrun-and-pseudo-random-domains/ the problem is Plesk-specific and brings up an interesting issue. If hackers are gaining access to Plesk, and the servers have already been patched, then either:

I've changed only my admin password for now, and emptied the session table. Waiting to see it it happens again.
Note that on Windows the SQL or MySQL password is really hard to find.
I also have the complication of PBAS and Expand that prevents me from upgrading my Plesk installs.
My plesk admin site log is full of lines like this:

I have a server that didn't install properly. The install won't complete and I can't uninstall. Is this for real? I was writing uninstallers back in the 90s.
Can I bill Parallels for my time to drive to the data center and reinstall Windows?

Opened a ticket to report this. Was offered no solution or workaround, but told that during the next release I should manually run the installer with a flag to prevent any further permissions changes.
I have another ticket open now. Hopefully they will be helpful this time.

We've also had the plesk SQLEXPRESS database refuse to start because psaserv had an ACL of Deny Everything, ahead of NETWORK SERVICE.
I just found a domain whose home dir c:\inetpub\domain.com doesn't have permission for the user or the IUSR_user so they can't log in...
Not 100% certain if it's related but the Dimac Jmail control stopped working at around the same time. Permissions were set to deny access to group psacln, which of course broke it.
Me too. Windows 2003. It started back in July when I ran the Plesk reconfigurator to repair disk permissions. A few sites lost their anonymous access.

Mine is doing it too. Just upgraded to 8.4 a couple of days ago and it was ok until tonight. From the error log, SQL EXPRESS is running as NETWORK SERVICE which is denied access to the data folder by the Deny ACL for the psaserv group.
