CentOS 6.7 (Final) MySQL 5.5.44 This system comes from Heart Internet both the VPS and Dedicated Server so it sounds like the templates could be the issue. As I think they must install same setup to VPS and Dedicated Server - this would explain why both are doing the same.

I have found port 3306 MySQL closed I have tried iptables -A INPUT -p tcp --dport 3306 -j ACCEPT But doesn't seem to work. Any ideas?

Just tried copying a database from a dedicated server using Plesk 12 to a VPS server using Plesk 12 and still get "Unable to connect to the database server. Please check the database user credentials and access restrictions." So wondering is there more to the IP address than I have put?
