Mysql Service running at 93% CPU!

MySQL is eating my server alive. It is consistently at 93% or so.

Mysqld-nt.exe is the specific process.

It is not related to specific high-traffic times, it just seems stuck. I have pretty optimized code on all my sites, I can't imagine that it could be leaking bad enough to cause this.

Can someone assist?
