postfix gives dsn=4.3.0, status=deferred (Internal mail system problems )

Having problems with a Plesk 9.2 install using Postfix.

On some accounts, mail does not deliver, but instead logs something like:

Oct 9 01:36:56 servername postfix/pipe[24328]: 1F5B313103DC: to=<user@domain.tld>, relay=plesk_virtual, delay=0.04, delays=0/0/0/0.04, dsn=4.3.0, status=deferred (Internal mail system problems )

Any ideas as to what might be causing this? Postfix has much better logging than the horrible Qmail, but it's still not very helpful.
 
We have the exact same problem after switching from qmail to postfix. Anyone have a solution?
