I think I found the problem, and its the WordPress Tool Kit. I have removed the extension and the [[customButton]] disappears.

I have removed 21 and 22, but I still have the user cp [[customButton1]] showing up. Restarted everything.

Why am I seeing a new menu option in the control panel top nav menu? [[customButton1]] I haven't added any custom buttons at all.

We have the site working agian now, there was a problem with the .htaccess file and a plugin "W3TC minify" rewriting urls. Even though the plugin had been removed. But my question still remains, when the site was backed up it was working perfectly and for some time, after a restore it was not?

Am I missing something here, surely restoring everything should put a site back to its original configuration? And not leave it in a broken state
