I solved the problem downgrading apache_control_adapter script to the version included in Plesk 12.5. No more Apache restarts after graceful.

Same problem here, even with the patched apache_control_adapter. Plesk servers with a few domains are working fine. Plesk servers with about 200 domains are having problems, it seems to make a graceful and then a restart. Is it possible to have an uncompiled version of apache_control_adapter...

Same problem after upgrading 12.0.18 to 12.5.30 with fail2ban (upgraded from EPEL) on Centos 7. The solution at this moment is remove fail2ban before upgrade. Please fix it.

Same problem here after applying last Microupdate in Plesk 12. I solved the problem with http://kb.odin.com/en/221 but I cannot select spanish languange anymore.

I solved the problem restoring the file /var/lib/plesk/mail/handlers/handlers.db and the /usr/local/psa/handlers/ directory with a backup made before the upgrade. I don't know if this is a "safe" solution.
