On Plesk for Linux mod_status is disabled on upgrades to improve Apache security. This is a one-time operation that occurs during an upgrade. You can manually enable mod_status later if needed.
Still experiencing this exact issue on Plesk 17.5.3 Update #61 with phpMyAdmin 4.6.6 using PHP 5.6.
My solution is to change the name of the database user.
(If the user name gets changed back to the old value, the bug reappears.)

Mailboxes can be created with an empty password if the checkbox...
I can reproduce the bug now as follows:
- Click on "Create Email Address"
- Enter something into field "Email address"
- Uncheck "Access to the Customer Panel"
- Click on "OK"
The mailbox is created without a password. The culprit is that the customer panel access is unchecked. Please confirm.
I need to correct myself. Can't reproduce this behaviour on a vanilla Plesk Onyx installation. I have a legacy system where the bug occurs as described, but I'm going to look into it further and mark this as resolved for the time being.
I marked this thread as unresolved as this behaviour still exists in the latest Update #41 on Plesk 17.5.3.

Got it figured out. I was initially using the same mailbox for sending that test email, so using local delivery. It looks like the "Received" header won't show up then. When using a different, non-local mailbox to send the test email, the "Received" header shows up just fine. May this be of use...
