I fixed the problem by enabling ASP.Net on the domain.
For the sake of security, I prefer to disable technologies that aren't being used, but it looks like if your using shared SSL ASP.Net is a required technology.

I'll try that next time. Today I just deleted the domain and recreated it, it was just a utility domain that I had created for shared ssl & plesk access so it didn't actually host a website or email.
If it were a customer's domain I wouldn't have been able to do that though, so...
PS. I've already disabled Shared SSL via Home > Tools & Settings > Shared SSL
I've also deleted the certificate from Home > Tools & Settings > SSL Certificates
Yet it's still there when I go into the domain's Plesk panel.

OK I have a partial fix for this problem ... somehow the license key had disappeared so we were running on the lite version of SmarterMail that doesn't support IMAP. I re-entered the licence key and IMAP is working again.
So now we just have the Horde authentication problem ....
