Question E-mails you think you've sent, but... surprise in the stats?

Frankly, am I the only one who sees weird stuff with email deliverability. You've set up your SMTP, your DNS and everything's running smoothly... or so you think, but when you take a closer look at the stats, there are black holes . It's like, mails are supposed to be sent, but no trace in the reports. What's the cause?
