Local queue in Qmail and mail with the subject Viagra

Good night. The local queue of qmail is growing and the messages are not being delivered in Maildir accounts. What should I do?

Also, is showing a lot of emails with the subject Viagra, coming from registered accounts on the server. How can one redirect email address send these messages?

The rules of spamassassin are updated. How can I solve the problem?
