Outgoing mail limit stats not updated

How are the messages counted? I have an issue (Plesk 12.5 on Linux / CentOS 6 with Postfix) where some outgoing mail (which happened to be from a spammer!) was not counted. The domain showed 0 outgoing mails in stats, but the maillog has the following (small extract):

This is repeated for 1000's of messages yet not one of these appears in the outgoing stats!
Meanwhile, for other domains I do see outgoing messages tracked normally.
Why are the above messages on a particular domain not counted correctly?]
Are there any logs for Outgoing Mail Control to track what it is doing?
 
Further to this - it seems that only forwarded mail (mail forwarded to an external email address) is being counted in the stats. Mail sent through an SASL-authenticated SMTP connection (the only other sort we allow) is not counted in stats.

In all likelihood this is a configuration issue, but I am stuck on trying to narrow this down further as there is virtually no documentation on how the Plesk Outgoing Mail Control module determines the outgoing mail stats.

Any help to point me in the right direction would be hugely appreciated!
