Qmail FreeBSD Random Breakages

Just alerted to a problem from a client, Plesk is randomly not adding domains to rcpthosts when domains are added. This box has been up for about a month, and has about 40 domains we've added, 7 of them do not have entries created in rcpthosts and they have been bouncing mail, even though mail is enabled, and all accounts are created properly. I went ahead and added 6 domains testing adding each the exact same way, adding the domain then creating one regular mailbox account, in 1 case Plesk did not make the rcpthosts entry.

This machine is running 7.5.2 on FreeBSD 5.3-Release p8.
