qmail-smtpd high cpu

Loads of qmail-smtpd processes when running top and load averages between 10 and 20, anyone know what the cause is, bearing in mind i have both dh.pem versions in my control file?

Is there many mailboxes on the server? They can be used heavily, in this case the load average can be high. Does mail service work as it should? Do you see any errors in /usr/local/psa/var/log/maillog ?
