In the default install we had a problem with attachments being attached as 0 bytes (or close to it). We have to change the PHP temporary files directory (PHP.ini) and assign permissions properly to get it resolved.
You should open your PHP Ini and make sure the line which contains the Temporary files directory points to a directory which has permissions for the IIS User (IUSR) to write / read files. Our problem was that it was set to a path that did not exist on the server.
