Single Mail Account won't use individual settings for spam

I have a single mail account which is incorrectly using the server-wide settings for SPAM management. Other mail accounts on the same domain correctly use the individual settings.

I have turned off and then turned back on using individual settings.

I have checked on the file system that the files and directories under /var/qmail/mailnames/[domain]/[account] have the same permissions and contents as the ones for an account that is working correctly.

I have stopped and started the server and the mail service multiple times.

I am using postfix on Plesk 9.5. The system was upgraded from a much older version of plesk using qmail, and the account with the problem is an upgraded account. Other upgraded accounts don't appear to have a problem. Does anyone have any idea what else I should check/change? Is there other info I should post here to obtain help? Thanks in advance!
