SMTP stops how do i find whats wrong

This has happened twice in 2 days but and in mail.err i get entries like:

Apr 24 02:18:08 2 qmail: 1145837888.734603 alert: cannot start: qmail-send is already running
Apr 24 03:58:35 2 qmail: 1145843915.337699 alert: cannot start: qmail-send is already running
Apr 24 05:39:02 2 qmail: 1145849942.382354 alert: cannot start: qmail-send is already running

i can just manually start it again in psa. Any ideas to determine the cause?
