After upgrade to 10.3.1: Fatal error in /opt/psa/admin/plib/Os.php on line 16

After upgrading to 10.3.1 I get this error in /var/log/sw-cp-server/error_log whenever I try to create a new domain or subdomain:

2011-08-09 09:44:54: (mod_fastcgi.c.2588) FastCGI-stderr: PHP Fatal error:  require() [<a href='function.require'>function.require</a>]: Cannot redeclare class adminpassword in /opt/psa/admin/plib/Os.php on line 16

System is Ubuntu 10.4 LTS 64-bit.
"uname -a" returns: Linux host.tld 2.6.32-33-generic #71-Ubuntu SMP Wed Jul 20 17:27:30 UTC 2011 x86_64 GNU/Linux

PHP info:

PHP 5.3.2-1ubuntu4.9 with Suhosin-Patch (cli) (built: May  3 2011 00:45:52)

How can I resolve this so that I can create domains & subdomains for my clients?

Looks like upgrade was not successfully completed. Did you try bootstrapper repair?

Yes, but that did not fix the problem.
