plesk panel is not accessible after update to 12

After update, my plesk panel 12 for windows is not accessable any more.
its giving this error

ERROR: PleskMainDBException: DB query failed: Unknown column 'description' in 'field list' (db.php:61)

please help

also
health monitor, sql server, plesk website service not starting.

Looks like upgrade procedure was not completed successfully and some database records were not updated. Try to run upgrade in CLI with

ai.exe --console

and complete upgrade. Ask free assistance here - http://odin.com/products/plesk/free-migration-support/ if it not help.

i follow ai.exe --console but no help. the issue remains.
