Valid eMail stuck in Mail Queue

I've got some valid email that the server has captured in the "Mail Queue" that has to be delivered. We are not running any spam killer programs so there is no obvious reason why the system has siezed it. How can I approve the delivery of this mail and how do I make sure that it doesn't happen again?
