Web Server restart everytime I make changes (Bad Gateway)

Every time I make changes i.e change a password for a user or update details the server restarts and I get a "Bad Gateway" error any idea what could be causing this?

I try run that query I get -bash: syntax error near unexpected token `('

And I also cant get to mysql as root it just says access denied.
 
It is not bash but sql command. If you have Plesk 11.5 version you can use following command for login to Plesk psa database:

# plesk db

For other Plesk versions use command:

# mysql -uadmin -p`cat /etc/psa/.psa.shadow` psa
