Hello!

Firstly, please try to reset browser cache for WordPress Toolkit screen by pressing CTRL+F5.
If that doesn't help, try reinstalling the WordPress Toolkit using the CLI command (it's not need to remove WordPress Toolkit before):
plesk bin extension -i wp-toolkit
And If that doesn't work either, contact Plesk Support.
