Database roles reset

Plesk 9.2 | MSSQL server 9.0.3042

It seems every night for the past few days, one of the client's on this server has all their database user roles changed from what they were (even from db_owner) to db_denydatareader and db_denydatawriter, thus breaking their site and SQL management studio access. They have three users setup through the plesk interface, and all are reset to these deny permissions.

It only seems to be this one client. They swear up and down that they have made no change to their website that would prompt this. Has anyone had any experience with this?
 
Also, perhaps unrelated, whenever this happens, the FTP user password is also changed and they can no longer get in.
