Emails lost between arrival and inbox

This started as a problem receiving email sent from a PHP script on another server, but it seems to be more than just the automated mails.

A client was complaining that none of their email addresses were receiving email sent from a particular PHP form submission script. We grepped her mailbox for the subject of the sample message, and found it, but she is not seeing it in webmail or when she downloads her mail through POP.

We have also tried setting up forwards at two domains on different servers, and mail sent through those is also lost.

I have tried adding the domain to both the SpamAssassin whitelist and the whitelist in the Plesk Mail panel, but so far nothing has arrived.

Any suggestions?
