Health Monitor E-mail Using All Parts Of Admin Name as sender

In Panel V11.5.30 when I receive Health Monitor e-mails, the sender ("From:") field lists multiple identities which are my Admin "contact name" split up such that one entity will be my First name and another entity will be my Last name, and both of these appear twice so in reality - there are 4 "senders" listed. One pair is associated iwth the "From:" field and the other pair is associated with the "Reply-To:" field (2 First name entities, 2 Last name entities). I can't see where to fix this.
