Hosted domain connection refused - help...

Today all my client websites were offline until I restarted the nginx reverse proxy server! Please Plesk Dev Team this is a huge BUG you guys need to fix!
 
Hi two days ago, I migrated Plesk 12 to Plesk 12.5 on a new clean install Centos 7. If click on certain forms and pages that produce denied access Apache error, my ip address gets blocked for about 20 to the entire server. The only difference is that the site remains accessible from a different ip address every time this happens as I have to keep on working on a bunch of Worpress sites on different domains I manage. My question is how does Apache/Nginx decide to block an ip address.
