Feedback Form issues

We have clients receiving blank feedback form emails.

Even though all fields are compulsory and when the form is visited in a web browser the error "Please fill in all the required fields."

comes up when attempting to submit a blank form, they are receiving blank email, with the from field being user@servername. which is confusing them.

Having a look at the Apache logs shows no visitors at the time the form was submitted. Is there another way the form might be being triggered in error?
