Editor Not Showing Images

I've created a new simple template table layout (using one of the example templates given) however when this new template complied and uploaded, the Sitebuilder Editor doesn't show and of banner images, even though the naming conventions are the followed ? Is there something I'm doing wrong or this a bug ?

I want to know if it is possible to design templates in DIVs rather than tables. I've done a few which display correctly on the Design Template Preview Tool v4 but not in the SiteBuilder Editor, is there any reason for this? If we cant design in divs for 4.2, can they be done for 4.5?
